First, if you have participated in the Mammoth Parade in the past, we would like to THANK YOU for your involvement. Many of you have participated for many years in a row and your support is greatly appreciated.With your help we have created an event that demonstrates the Patriotism and excitement that our small towns exhibit.

This year, we hope to make it even more enjoyable for the citizens who will come to watch. To that end, we are sending you this application early so that we can try and meet the needs of the participants.

Parade Rules (please read and initial or sign where indicated)

Arrive by 2:30 and check in at the North side of the Swimming Pool building on the North side of theCity Park. Make sure that you have completed all paper work and have received your map.
•Move to your assigned starting position. At least one person must remain with the entry at all times.
•The parade will start at exactly 4:00 pm so be prepared. If no one is there to move the entry, then the others will move around you and the parade will continue.
•The City in cooperation with the State is no longer allowing candy or other items to be thrown from the vehicles or refilled from the vehicles. If you wish to disperse candy or other items you will have to provide walkers with buckets or wagons to walk along the WHITE solid line& throw towards the side walk. They must be supervised and may not refill from the vehicle so plan accordingly.
•Above all, understand that safety is our priority. The Law Enforcement and Parade officials have ultimate discretion and will remove any entry for violations along the Parade Route.

Waiver of Claims and Release of Damages for Participation in an Approved Special Event (Absolutely required for Participation)

Each of us individually( and /or as parent and/or guardian of the named ) do hereby release, remise, waive and forever discharge the State of Utah, Utah Department of Transportation, Utah Transportation Commission, Utah Highway Patrol, Nephi City, Nephi City Police Department, Juab County Sheriff’s Department, Juab County, and their officers, agents, volunteers, and employees from all liability, claims, demands, actions or causes of action whatsoever arising out of or related to loss or damages and/or injuries, including death, which may result from my participation in the MAMMOTH PARADE.
